Out-of-Home Residential Support Services
Residential Support Program
Intermountain has been providing residential support services to Developmentally Disabled individuals in Yavapai County since 1978. Referrals are accepted from the Arizona Department of Economic Security/Division of Developmental Disabilities, tribal Social Services agencies, and private pay resources. Currently the residential program offers individualized support services in six group homes and six developmental homes for adults who have been diagnosed with Developmental Disabilities. The program, designed to provide positive support for individuals with disabilities so severe that they can not live with their families, operates in the least restrictive setting possible and encourages as much independence and community involvement as possible for each resident. Teaching and support is provided within the context of the daily living routine, in a family-style setting. The program design is individualized for each participant, according to the client’s strengths and needs. The results of each resident’s comprehensive functional assessment are utilized in building their plan for life skill acquisition.
All of the residential program support provided in the group homes and developmental homes is designed to increase self-sufficiency and foster a strong sense of self-worth in each individual. Teaching and training skill areas consist of five primary domains:
- Self Care
- Community Living
- Academic and Vocational Skills
- Recreation and Leisure Skills/Activities
- Social Skills and Communication
Group Homes for Adults
Two and sometimes three Developmentally Disabled adults typically dwell in each group home, with one or two staff 24 hours, 7 days a week. All of the group homes are located in either Chino Valley, Prescott Valley or Prescott. Service is available to individuals who are 18 years and older.

Developmental Homes for Adults and Children
Designed to resemble foster care for children, each developmental home is designed to operate for the benefit of individuals with special needs due to the diagnosis of Developmentally Disabled. Intermountain identifies and trains community members in Yavapai County to provide these services in their homes.

Success Story
Cindy, who is Developmentally Disabled, came to the Intermountain Chino Valley Adult Program in July 2001. Four major obstacles hampered her ability to live independently:
- Limited Self Confidence
- Limited Community Involvement
- Poor Anger Management
- Physical Side Effects of Medications
Living in the Intermountain group home“Lomadufki”, Cindy was introduced to and has developed interest in areas that she had never previously been exposed to:
- Applying Make-Up and Personal Hygiene
- Acting in Community Skits and Programs
- Modeling in Local Fashion Shows
- Mastering Computer Skills
- Mentoring Other Adults in the Day Program
- Learning the Importance of Good Nutrition
- Preparing Healthy Snacks for Her Peers
- Exercising Daily
- Losing Weight
Cindy has taken a TOPS (Take Off Pounds Sensibly) class and voluntarily shares her new knowledge by preparing healthy snacks for her peers. Due to Cindy’s new found interest in and dedication to nutrition and fitness, she has now lost 35 pounds.
Cindy’s new leisure skills, developed in the Intermountain Adult Program, have built her self confidence and provided her a positive stature in the Prescott/Chino Valley community. After several months of volunteer work in the local DES/DDD office in Prescott, she was offered a permanent position and enjoying her work there immensely!
