Northern Arizona Programs for Developmentally Disabled Individuals
Intermountain Centers for
Human Development has
been providing residential
support services to
developmentally disabled
individuals in Yavapai County
since 1978.
Operating from offices located in Chino Valley,
Intermountain’s programs for residents of
Northern Arizona currently serve individuals with
developmental disabilities both in small group or
family residences and through the Day Program.
All services are developed and delivered following a strength-based model, designed to build positive and productive self-management and practical life skills of the clients which increase their self esteem and independence. Referrals are accepted from the Arizona Department of Economic Security/Division of Developmental Disabilities, Tribal Social Services agencies, and private pay resources.
Out-of-Home Residential Support Services
Intermountain offers a continuum of out-ofhome placements for children and adults whose function is so impaired that they cannot live with their biological families. Currently the residential program offers individualized support services in nine group homes and ten developmental homes for adults who have been diagnosed with developmental disabilities.
The results of each client’s comprehensive Functional Assessment as they enter the program are utilized in building their plan for life skill acquisition and determining where the client might be best served. Whether delivered by direct care staff in the group home or by “foster families” in the developmental home, intensive programming is provided in the least restrictive setting possible, encouraging the development of positive life skills and community involvement for each resident. Teaching and modeling occurs within the context of the daily living routine, in a family-style setting. The program design and services rendered are individualized for each participant, according to the client’s strengths and needs.
All of the residential program support provided in the group homes and developmental homes is designed to increase self-sufficiency and foster each individual’s sense of self-worth. Teaching and training skill areas consist of five primary domains:
- Self Care Skills
- Community Living Skills
- Academic, Vocational and Practical Life Skills
- Recreation and Leisure Skills
- Communication and Social Skills
Group Homes for Adults
Furnished and operated by Intermountain,
each group home is a residence for two to three
developmentally disabled adults, with one or
two staff present 24 hours, 7 days a week. All
of the group residences are located within the
tri-city area – Chino Valley, Prescott or Prescott
Valley of Yavapai county. Services are available to
individuals who are 18 years and older.
Developmental Homes for Adults and Children
Designed to resemble foster care, each developmental home operates for the benefit of individuals with special needs who would profit from a residential teaching environment located within a family. Intermountain identifies and trains community members in Yavapai County to provide intensive support services in their homes. Intermountain staff members monitor and support the individualized program with weekly visits to the residences.
Day Program
Developmentally disabled individuals attending the
Day Program are both residents of Intermountain’s
group and developmental homes as well as community
members who reside at home with their families. The
Day Program participants spend five days a week in a
relaxed educational setting where they receive both
individualized support of skill development mandated
in their Support Plan, and stimulation of leisure and
recreational interests through enjoyable group activities.
Each client’s strengths and needs are assessed upon entering; and their individual goals are set by their person-centered team. Intermountain Day Program staff provide positive reinforcement for each client in order to support the acquisition of positive practical life skills, to increase participation in daily self care and family responsibilities, and to build communication and pro-social skills. The development of self-management skills and self esteem are universal goals for all participants.
The Day Program site is located in rural Chino Valley,
a pastoral property with easily accessible outdoor
spaces. A maximum of eighteen (18) developmentally
disabled participants attend each day.
The daily programming is multi-faceted and incorporates a wide spectrum of activities that are educational, vocational, and recreational, depending on each individual’s goals. Learning opportunities might include acquisition of communication skills for non-verbal clients to expansion of vocabulary for verbal clients, mastery of the alphabet and counting numbers, typing and navigation on a computer, money counting and transferring, manipulation of art media, making music, and interacting with strangers in the community. Field trips to community-based events, appointments and visits to local eateries for lunch augment the routine designed to collectively and individually benefit the clients.
The hours of operation are 9:00 a.m. to 3:00 p.m., Monday through Friday. Referrals are made through the Department of Economic Security/ Department of Developmental Disabilities.
